    • Submit a subject Access Request (SAR) under the Data Protection Act
    • Make a complaint about a member of our team or the service you have received from our office
    • Request SCOAF promotional material
    Do not use this form to make enquiries about your application form. You should send your enquiries to the contact mailbox at contact@scoaf.org.uk. If you want to make a complaint, you should complete an Application for Referral.
