Are you a respondent in a Service complaint, or have you supported a respondent in a Service complaint? Then we’d like to hear from you about your experiences and whether having access to the Ombudsman would’ve helped. For more information click here.

I want to make a formal Service complaint

There are two ways for you to make a complaint.

You can:

  1. Submit a written complaint to your chain of command.
  2. Ask the Ombudsman to refer your intention to make a Service complaint to your chain of command.

The Ombudsman can’t investigate your Service complaint at this stage. It must go through the internal complaints process.

Your complaint might have to go through a separate complaints process first – see Special-to-type matters. This will depend on what you want to make a complaint about. Your chain of command will let you know if this is the case.

If you want the Ombudsman to  make a referral, click the button below to access the application form.
